Webinar Purpose:

To provide participants with the skill sets necessary to implement and manage a clinical case management model.

Today’s hospitals are challenged with managing patient throughput due to increasing regulatory and quality issues. Although, many hospitals have implemented case management processes to meet this challenge, some have had limited success in meeting their targeted goals of length stay, effective resource utilization and quality outcome measurements. This program will provide participants with the structure, processes and documentation tools for an effective approach to managing patient throughput. Strategies to align physician support for the concept will also be shared. Taking into consideration, that each hospital needs to implement a model that aligns with its vision and political atmosphere, the models presented are representative of hospitals ranging from 50 to 900 beds.
